I work with a wide range of emotional and behavioral issues, providing services that span from therapy for depression and grief counseling to parenting support, couples counseling and beyond. In a comfortable and supportive atmosphere, I offer a highly personalized approach, tailored to each of my clients individual needs, to help attain the personal growth they’re striving for. Therapy is designed to help you to find your own direction and path.

Practice Limitations

I limit the scope of my practice to my training and to those individuals I believe that I am best qualified to serve. I will refer out to other specialists, for below mentioned situations/cases,
You will be asked to sign this disclaimer upon starting therapy.

Types of Treatment NOT provided by this office:

–Any court ordered treatment, unless thoroughly discussed and agreed upon, prior to treatment, by the therapist, the patient and possibly the courts.
–Any treatment provided to Sex Offenders or Offenders of Domestic Violence.

–Any and all legal cases in which the therapist will be asked to testify; including, but not limited to; Child custody, divorce, any civil cases.

–Any type of treatment considered to be within the sub-specialty of  “Sex Therapy”, which would also include porn addiction and transgender issues.

–Workman’s Comp cases.

–Therapy aimed at gaining Social Security Benefits.

–High Risk patients (including; actively suicidal, homicidal, recently hospitalized, multiple suicide attempts or psychotic people.) Such patients will be referred to an office with a Psychiatrist present.
–Substance abuse cases in which the patient is not actively involved in and/or refuses a 12 step program.

–Anyone who is not a willing participant in the therapy process, including underage persons. (Minors under age 18 will be provided with two sessions in order to ascertain whether or not they wish to continue in the therapy process. If they prefer not, they will be referred out to a therapist who is willing to work with non-compliant patients.)

–Patients non-compliant with medication prescribed by their Psychiatrist and/or Physician.

–Patients non-compliant with treatment recommendations and who require frequent rescheduling of appointments.

— Prospective patients whose treatment would knowingly create a conflict of interest, by seeking therapy with this therapist. (examples can include, but are not limited to; ex-significant-others, of present or past patients, significant-others of patients seeking divorce or separation, and persons in conflict with past or current patients.)
If discovered after treatment occurs, referral will be made to another clinician, based upon “conflict of interest”. No mention of the other patient’s name will be given to the patient being referred out.

Paula M Porterfield-Izzo, MA, LMHC, LPC, NCC will refer any of the above situations to an appropriate Specialist.

Insurance

Most insurance companies cover Mental Health Counseling Services, with you sometimes paying a co-pay. Some plans have deductibles, but not all.

Services may be covered in full or in part by your health insurance or employee benefit plan. You are ultimately responsible for full payment.

Please contact your insurance company and check your coverage carefully by asking the following questions:

  • Do I have mental health insurance benefits?

  • What is my deductible and has it been met?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount per therapy session?

  • Is approval required from my primary care physician?

Customary Fees

The fee for an intake is $125 for a session, which lasts between 50-60 minutes.

The fee for follow-up counseling appointments is $100 per session.

For example, if an individual were to seek counseling once a month for a year, they would expect to pay $1225 total for the year, which would include the initial evaluation.

Reduced Fee
Reduced fee services are available on a limited basis, and only by prior arrangement.

Payment
Due to the distance nature of Telehealth Counseling, all major credit cards are accepted for payment. If you prefer to pay in advance by cash, arrangements can be made.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session.
